Herefordshire Children’s Community Nursing Service – Supporting Health Needs at Home and in School 

 

The Children’s Community Nursing (CCN) Service in Herefordshire provides specialist nursing care for children and young people with acute, complex, or long-term health needs. The aim is to help children live as well as possible, whether at home, in school, or in respite care, while supporting families and reducing the need for hospital stays. 

 

Who Is the Service For? 

  • Children and young people from birth up to 18 years old 

  • Young people up to age 19 if they attend a specialist educational setting 

  • Must be registered with a Herefordshire GP 

  • Must have an identified nursing or medical need 

 

What Support Is Offered? 

The CCN team provides a wide range of care and support, including: 

  • Managing medical devices and procedures (e.g. gastrostomy, nasogastric feeding) 

  • Wound care, including after surgery or injury 

  • Training for families and school staff to safely carry out medical procedures 

  • Bladder, bowel, stoma care, and specialist continence support 

  • Palliative and end-of-life care, including coordination with respite services 

  • Support with transitions from children’s to adult health services 

The team works closely with families so they don’t have to manage complex care alone.

  • They can train school staff to carry out care tasks safely and confidently
