Transition to Adulthood: Support from Herefordshire Council’s Young Adults Team
As students near their 18th birthday, important changes take place in the support they receive. At Accordia Trust, we work closely with Herefordshire Council’s Young Adults Team to ensure this transition is smooth, well-informed, and focused on each young person’s future.
What is the Young Adults Team?
The Young Adults Team is part of Herefordshire Council’s Adult Social Care service. It supports young people aged 18 to 25 who have eligible needs under the Care Act 2014. The team helps bridge the gap between children’s services and adult services, promoting independence, wellbeing, and personal goals.
They work in partnership with:
Children With Disabilities (CWD) Team
Special Educational Needs (SEN) Team
Health, housing, employment, and day services agencies
Who They Support
The team supports:
Young people aged 18–25 with eligible care and support needs under the Care Act.
Those with an Education, Health and Care Plan (EHCP), transition planning begins from age 14, with reviews from age 16+ attended by the Young Adults Team.
What Support Is Offered?
Support is tailored to each young person’s needs and goals, and may include:
Assessment & Planning: A strengths-based assessment around 6 months before turning 18.
Tailored Support Plan: Covering areas like personal care, independent living, education, employment, and community inclusion.
Multi-agency Coordination: Linking services across health, housing, education, and more.
Advice & Signposting: For those not eligible for formal services, guidance and referrals are still available.
Annual Reviews: To ensure support remains appropriate and effective.
What Families Need to Know

How to Access Support
Speak with your class teacher or EHCP lead well before your child’s 18th birthday.
Attend your Year 11 EHCP review (and earlier ones) to help gather information for your adult support plan.
After turning 18, if you believe you have support needs, you or your family can request a Care Act assessment via Adult Social Care.
